Problem Statement & Community Stakeholders
Sexual health education is a crucial aspect of education because it provides adolescents with information that will allow them to enter adulthood more fully prepared and informed. Health education involves multiple facets: emotional, social and physical health. Sexual health is a compilation of all three of these areas. In Multnomah County, there are great disparities that exist in the adolescent population in terms of sexual health outcomes. These disparities are most noticeable when comparing the African-American adolescent population to White students of the same age. There has been an increase in sexually transmitted diseases and teen pregnancies in recent years in Multnomah County; these numbers are especially high among the African Americans.Public health officials can play an important role in addressing this health disparity by advocating for policy level change and introducing additional prevention programs both in the classroom and in alternative settings. One example of a successful program aimed at addressing these concerns within Multnomah County is an educational program called “Adolescents and Communities Together Project” which is
Public health guidelines support the need for increased sexual health education. When considering the CHIP and CHA for Multnomah County, it is evident that the African-American community is especially in need of increased services and education. Public health data indicates that many of the disparities exist due to a lack of access to quality education and health resources. The goal of a new prevention program would be to build on existing public health efforts and fill in the gaps in care and services. The primary focus of the prevention program would be to continue reducing STI transmission among the African-American community and improving BIPOC adolescent sexual health outcomes.
In terms of poor sexual health outcomes, 15-24 year old minority groups are the most largely affected. According to the CDC, of 19 million new cases of STI’s that occur each year in the U.S., approximately half of those cases occur among people aged 15-24 years old. Part of this is due to poor sexual education and also lack of access to effective healthcare and protection methods. BIPOC communities are disproportionately affected by STI’s and the factors referenced above play a large role in the reason for the disparities. Racial and ethnic minority populations often fear and distrust healthcare institutions, which negatively affect their healthcare-seeking experience. This fear is built from a history of racial discrimination and violence. Over the years, many minority populations have faced unethical medical treatment or dismissal over legitimate medical issues. Because of this concern, prevention efforts needs to be targeted and culturally sensitive in order to meet the communities needs.
In Multnomah County specifically, there is a similar pattern of racial and ethnic minority groups struggling with sexual health. According to the Multnomah County Health Department, African Americans in Multnomah County are most likely to face health disparities among all races and ethnic groups. More than half of African-American youth do not complete high school, compared to just over a third of White students. This is important because most sexual health programs currently available only occur during high school. Black youth are also 6.1 times more likely to be charged with a crime than White youth, and are 33% more likely to be held in detention. Due to the high number of African-American students who are not in traditional classrooms, an important aspect of a prevention program would be the flexibility for it to be implemented in alternative settings. Access to sexual health education should not be limited to those who are able to be in the traditional classrooms because the benefits a quality program can provide are many. Sexual health education can improve academic success; prevent dating violence, and bullying; help youth develop healthier relationships; delay sexual initiation; reduce unplanned pregnancies, HIV, and other STIs; and reduce sexual health disparities.
